A comprehensive listing and description of the configurable settings available in the Form tool is found below.

Basic

  • Activity Status - Sets the form to either 'on' which actively accepts contributions from participants, or 'off' which does not accept contributions and advises participants the activity is closed. When a new form is added to a page, its status will default to the active ‘on’ state.
  • Activity Title - Provides a title for the form to identify it in the Reporting area of the Dashboard. If the 'Display title' option is checked, the title will also appear to visitors of the page. The title is also used in the confirmation notifications (if enabled).
  • Activity Description - Adds a description that provides further information about the activity in your Reports. If the 'Display description' option is checked, the description will also appear to visitors of the page.

Display

  • Form Type - Determines the way the form and its questions and content are displayed to your participants, with three choices available: Standard, Paginated or Flow (view the editor page for a more detailed description of each form type).
  • Question numbers - Lets you enable automatic numbering for each of the form's questions. This is useful when logic is used, ensuring the participant is always shown the correct number.
  • Page numbers - Turns on automatic numbering for each of your form's sections when using the Paginated form type.
  • Progress bar - Adds a visual 'progress bar' to the form so participants can see how far along they are in completing it. This option is only applicable in the Paginated form type.
  • Prev Button Label - Overrides the default text for the 'Previous' button which is used by participants to navigate back through a form. Text is limited to 20 characters. It is only applicable for the Paginated and Flow form types.
  • Next Button Label - Overrides the default text for the 'Next' button which is used by participants to navigate forward through a form. Text is limited to 20 characters. It is only applicable for the Paginated and Flow form types.
  • Submit Button Label - Overrides the default text for the 'Submit' button which is used by participants to submit their form on completion. Text is limited to 30 characters. It is only applicable for the Paginated and Flow form types.
  • Completed Message - Overrides the default text for the standard 'success message' which is shown to participants after they submit a form. By default, the message reads 'Thank you for your submission'.

Advanced

  • Unique Contributions - Ensures participants can only make a single contribution to the form and cannot complete it more than once. This feature requires your participants to be registered Members and does not allow anonymous contributions. By enabling this setting, you will also automatically turn on the 'Registration Required' setting (see below).
  • Registration Required - Determines whether participants can be anonymous or must be registered Members to make a contribution. If the setting is turned to ‘on’, participants must complete the registration process before making a contribution, and a notification will be displayed at the top of the form prompting visitors to login or sign-up.

Notifications

  • Contribution Notifications - Sends notification emails to nominated users each time a form is submitted.
  • Email confirmation - Sends an email notification to the participant to confirm their contribution along with a copy of their submission. To enable this feature, an Email question type must be added to the form.
